A completed, physician-signed medication authorization form is required for any prescription medication administered at school.

Submitting Health Forms

  • Work with your child’s primary care provider or specialist to complete the appropriate forms.
  • Return forms to your child’s school main office (not through students’ backpacks if possible).
  • Update forms whenever there is a change in your child’s condition, medications, or treatment plan.
